Below is a list of documents your consultant will need in order to match your qualifications and preferences with the best opportunity available.  These documents may also be utilized when applying for hospital privileges and licensure.

A copy of your:

  • Curriculum Vitae
  • Medical School diploma
  • TTG Provider Agreement
  • Internship, Residency and Fellowship certificates
  • Medical Board certificates (if applicable)
  • Current state license card
  • ECFMG (if applicable)
  • DEA license
  • State controlled substance license (if applicable)
  • ACLS, ATLS, PALS, PACTS, PTLS certificates
  • Photo ID, this can be a passport and/or a clear copy of your driver’s license

In addition, completion of the following documents are required before the start of an assignment and the issuance of liability insurance.
